A Death Record is closed information and available to the public on and after the 25th anniversary of the date of death.

Requirements to obtain certified copies to qualified applicants:

  • Any person or organization that can demonstrate their legal relationship or their direct and tangible interest in a particular record is authorized to obtain a certified copy of the record.
  • The parent, child, brother, sister, spouse, grandparent, legal guardian or managing conservator of the registrant.
  • Legal representative, agent or other person acting under contract for the registrant must provide a letter of permission and a copy of identification from registrant.
  • The applicant must provide all necessary information and show proper identification prior to obtaining the certificate.
  • The hours of operation are Monday – Friday 8:00 am – 5:00 pm.
